Providence Horizon House

Years ago my uncle was a resident, it was great. No issues. He enjoyed it , until my parents came and got him. Staff friendly and food was good.

My grandparents and a couple other family members have had the pleasure of living at Horizon House the past few years. The apartments and dementia units all look spick and span. The staff are always friendly and inviting upon walking through the doors. They provide so many fun activities for the residence, including a Senior Prom! Additionally, Horizon House is very accomodating to both the residence and their families. Any time we have had concerns, staff is available to talk with us and walk us through procedures and the next plan of action. When we recently lost my Grandmother, my Grandpa couldnt afford to stay on his own and they provided our family with several resources to look into. I have had no issues concerning this facility and highly recommend it. Thank you staff for all that you do!

My grandparents have lived at Horizon House for several years now, and it's such a comfort to see that they're always well taken care of. I visit when I can, but it's nice to know the staff are always there to help and the other residents are good company. Thanks, Horizon House!

I just posted this review on Google maps. "As a Resident. I will say this facility is Corrupt to the core of care. The Administrator Thersa Gleason is a dictator of self-imposed rules, not Providence Guide Lines. I filed a complaint against staff and was served an eviction notice. The Nurse Jessica violated Hippa by contacting my Doctor and talking the Doctor into canceling orders the doctor agreed to with me. I was never contacted. If you have a service dog. Do not bring it to this facility. Providence Horizon House is not ADA-compliant with service dogs. Beth Musliu Rn. The nursing Supervisor takes it upon herself to set rules and guidelines for service dogs that do not comply with the ADA. This facility's CNAs are Attitudal and Lazy. This facility is an embarrassment to Providence and the care that seniors and the disabled deserve. A change of management is needed. If you file any complaints your deemed a threat to the facility and staff. I was served an eviction notice by email while at the hospital an hour before the scheduled surgery. A great job of hype and promises are made to get residents in. But once a resident is where you realize you made a mistake. Staff feel they can walk in your apartment anytime they want. They justify by saying "a concern for your welfare" but it's worse than that. I notified staff they need to ring bell or knock and wait for a reply. And that's where the retaliation and eviction notice starts. I highly advise you find your loved ones or self another facility until this unprofessional and unethical and corrupt management is fixed by Providence upper management"
