        Since May 05 of being submitted in to surgery at clinic Vascular Access Center at 11411 Brookshire Ave, to clear clogged fistula on my forearm, two days later I was admitted in to emergency to be install a cap on chest so I can receive dialysis treatments and days later mayor surgery and transplant of arteriovenous to repair damage cause by the attendant Dr.
