They sent me a bill in the mail but their phone number no longer works. Drove by the office and it is empty- no couches, no people, nothing.

I had my first cleaning scheduled for January 2021. I was not greeted in a warm manner and the office looks like it was last updated in the 1980s. They were short staffed that day and I waited a very long time to be seen. There was no hygienist so Dr. Beavin did my cleaning. She identified some cavities that needed to be filled and a tooth that needed a crown, which I was previously aware of. The cavities were filled at a later date with no issues. At another visit, Dr. Beavin prepped my tooth with a temporary crown. Less than an hour of my leaving the office, driving in my car with no food or liquid to speak of, the crown literally broke in half and fell out in my hand. I called the office and recall Dr. Beavin being genuinely surprised and saying, "Oh my gosh, I've never had that happen before!" (Many weeks later, she told me this was common.) The receptionist told me the next available appointment to fix this half broken crown WAS A WEEK LATER. She said that was all she could do. I told her that was unacceptable and I needed this fixed immediately. Dr. Beavin got on the phone and scheduled me 2 days later and replaced the temporary crown. Days later, I was in an incredible amount of pain in that area that got increasingly worse every day. This is when I learned that the office has very limited hours of operation: the voicemail says they are open Monday, Tuesday and Wednesday, taking an hour lunch break on those days. I called the emergency number and explained the pain I was in despite taking Tylenol AND Advil together. Dr. Beavin said it sounded like I needed an emergency root canal as soon as possible and suggested I call an endodontist. The next day, I found an endodontist that could get me in. That doctor did an initial exam and said I did not need a root canal. Then! My temporary crown fell out. I called the emergency number again and was advised by Dr. Beavin to get some adhesive from the pharmacy to hold it in place until the permanent crown was available. I used the adhesive several times over the next few days to keep the temporary crown in place. Eating and drinking was painful and I avoided it as much as possible. This experience to this point has been so upsetting and painful that I just wanted to cry, and often did. When I went to get the permanent crown on...it didn't fit. It had to be sent back to the lab. The receptionist also misscheduled my appointment based on our in office conversation. Finally, the permanent crown was installed with some adjustments. At my first visit, I gave the receptionist my insurance card. I was also asked to pay for the cleaning. The next visit, she wanted to charge me again. I asked why my insurance wasn't covering these things. She said that I had not provided her with my insurance info. Not true! Then she looked in her computer and indeed saw that she had my card on file and said she would file the visits with insurance. So, now the office had received my payment AND the insurance's payment for my first visit. Now I am trying to get a refund back and the office won't issue it, stating that I have an outstanding balance from the crown waiting for insurance to file. However, when the office filed the crown claim with insurance, they failed to provide any x-rays. The insurance requested the x-rays on April 19th. Dr. Beavin's office did not submit the x-rays to the insurance until May 24th, after I had called twice to inquire why this had not been resolved and request my refund for the double payment. This experience has been extremely unpleasant on so many levels. I do not recommend this office for dental treatment.
