I am new to the area so I rely on the list from the insurance company.  I secured an appointment at 8 AM and was told to arrive 10 minutes early to fill out paperwork.  When I arrived (10 minutes early) there was a gate blocking the entrance...that should have been my first clue.  A nice black lady  (worker) came over to open the gate.  I parked my car and walked to the door and it was locked ....hmmmmm (mind you, I was instructed to arrive early.)  3 minutes to my appointment time, I tried the door again and found it open.  I walked in, announced my name and was given forms to fill out in exchange for my ID and insurance card.  The forms asked for my date of birth and SS# as well as my husband's SS# and date of birth.  I didn't provide the SS# because who knows what kind of system this company has.  The person behind the desk told me she needed this information to process insurance (um, really!??)  I offered to provided my credit card for the charge but she didn't want it....she wanted out SS#!  STAY AWAY!!!!
