Forest Grove Rehabilitation and Care Center

I give this facility 3-1/2 to 4 stars. I was sent here followinng a 2 week hospital stay. My physicians and other caregivers felt that I needed more intensive nedical care and rehab/therapy than the assisted living facility where I'd been living could provide.i will however make some suggestions based on my observations and experiences here. Overall. The staff is stellar. Caring and responsive. But There are a few inconsistencies and some areas that could use improvement. The old adage that the left hand sometimes doesn't know what the right hand is doing applies. I feel the care provided could be.and should provide more coordinated care. There are times when it takes an extensive amount of time to respond to a call light. Perhaps more caregivers or better coordination and timing during peak times like shift changes and meals . And improved communication and scheduling all around,

My mom is here also. Thanks for all you do with my mom. I really like it here. I only wish it was closer to portland oregon so I could get to here more?

Iam a resident here and i have to say the staff here is great iam well taken care of here not everyone is going to have the best things to say about a nursing home but here your family regardless of the occasional miss understand and miss communication between dr and resident but if theres a mess up its fixed right away no excuses they have a verry caring director kayla and great DNS support social workers eh any way great place CNA'S ARE GREAT AS WELL

My mom was here for 7 days and the Manager of the Physical therapy dept. Didnt use a transfer belt on my mom and bruised her on both sides of her hips. I am very disappointed in the staff, cause they need to communicate better between each other, cause this could of been prevented! Get more training. My mom is still bruised. I wouldnt use this rehab for your care!

My son Douglas emery is a patient there. I spoke with him today and he seemed very upset. Said he couldn't talk..Kristen hung up on me when I questioned her about mandatory reporting and ombudsman
