Our model of care

We believe in whole health. This means we address and heal disease but equally important, we work at the causes of the causes, the social ills that must be addressed to achieve true equity.

Our Care Teams are designed to partner with our patients to meet their unique healthcare needs by integrating body, mind, and heart.

Union Community Care is a nationally recognized Patient-Centered Medical Home because of our approach to providing healthcare services that embrace each patient’s unique culture, needs, and values. Our inclusive Care Teams come to our community with their own individual experiences and diverse cultural backgrounds.

Our Board of Directors

As a Federally Qualified Health Center, 51% of our volunteer Board of Directors are patients of Union Community Care. This means we have an extraordinary combination of community leaders who understand and embrace complex lives and unique strengths and work hard to break down all barriers to care.

Our network of care

We provide primary family medical care, dental care, behavioral health, social support, urgent care, and pharmacy services throughout Lancaster & Lebanon. We offer a discounted rate or fee for medical and preventive dental services at our centers, 340B program savings, and a good faith estimate.
