The incredible hardworking staff in the facility is like no other! I am so proud of all the hard work done by all to always put the residents first as always!

I absolutely love my residents and the staff are family to me but the therapy team is outstanding. Enjoy my time there and will miss my unit 100

My father was placed in Autumn Care after sustaining a fall that required surgery and physical therapy. Per the advice of the orthopedic physician, we opted to use this facility for his rehabilitation, plus this facility had a 5 star rating. Our first impression was high. The individuals who worked here went out of their way to impress us...first clue! My father spent 7 weeks and 3 days at this facility. During his stay, he became both mentally and physically unstable. We even emailed the director at that time, to express concern. We were informed during the course of treatment, by the PT director himself that my father was progressing well, and was well beyond where he needed to be. On numerous occasions, we noted my father's clothes were soiled, he was not clean, and a large number of times he was unresponsive and confused. It appeared as if he was heavily sedated. My father could not eat the food, so we provided meals and snacks on visitation days. My father was expected to roll himself, in a wheelchair, to and from the communal restroom...yes the communal...he couldn't use the one in his room as his injury didn't allow it. His roommate experienced the same issues. My father had documented issues with both his eye sight and hearing and the staff was aware both had deteriorated over the course of his years. On most of our visits, x3 days per week, my father slept. Not a doze and wake. An unable to get a response sleep. This was extremely unusual, as he had never exhibited this behavior at home. All of his meds were given at one time, unsupervised. There were numerous times we found medication on the floor. During his stay at the SNF, he had 3 serious bouts of a urinary tract infection; 1 extremely serious; 2 bouts of cellulitis, conjunctivitis, and finally sores on his heels, which required diabetic boots. On all occasions we either brought it to the staffs attention, or we were uninformed until we were persistent in our questioning. We were told that both the physician and PA saw him on a consistent basis, but my father stated they had not, nor did we ever witness a provider there. We were told he was given medication for his UTI to later learn from the nursing director, even though it was documented, he had indeed NOT received it. We were also informed at his early discharge via PT director, per our arrangements, that he could transfer and prefer ADL with no issues. Therefore, a transfer would be an easy transition. In reality, he could not! The staff continuously ignored my father, unless we were there to track someone down. We witnessed it with both my father and other residents as well. When we confronted the staff, it was always, " We are short staffed ". My father came home on a Thursday and on Saturday at 2.30 am had a seizure. He spent 7 days in ICU on a ventilator and 7 days in a room. He was transferred to Carolina Caring and spent 5 days being cared for with love and compassion until he passed on July 8th. So, if you are cons idering a SNF, think twice. I wish we would have removed my dad....perhaps he'd still be here today. It is elder abuse,, to treat these residents this way. We also had residents to tell us the last week of my father's stay, " If you love your dad, get him out of here". We have tried to do a survey with Saber Health but have been unable to do so due to number unrecognizable per their facility. If we could rate this facility a negative 100, we would. It's rated high because staff members/employees are allowed to write and post reviews. Read the reviews written by patients family.....or suffer the pain of losing the one you love as we did. Note: phone calls to the Business Office Manager, Administrator and Social Worker go unanswered...even when messages are left! And for good reason: it's hard to answer for things done incorrectly
